**Job Description**

Provide support in implementing the patient safety risk management system.
1\. Collect and analyze information from monitoring activities and clinical record reviews to generate patient safety indicators, considering regulatory requirements and internal policies.
2\. Conduct surveillance and active searching for patient safety incidents through monitoring, clinical record reviews, use of recognized tools, and interviews with care providers in services and/or units.
3\. Integrate all sources of patient safety incident information: voluntary reporting, active clinical record review, complaints and claims; maintaining an updated database for generating information on incident management, which will be reported and disseminated within the institution.
4\. Promote and educate on institutional patient safety practices, approved and currently in effect; emphasizing Required Organizational Practices and comprehensive management of patient safety indicators.
5\. Participate in root cause analysis of patient safety incidents and events, as well as in developing and monitoring improvement plans derived from such analyses.
6\. Participate in patient safety measurement activities: supervisions, audits, tracer studies, safety rounds, and consolidation of obtained information.
7\. Actively participate in multidisciplinary working groups for developing continuous improvement plans or projects benefiting patient safety.
8\. Participate in developing continuous patient safety training programs, promoting adherence to approved and current patient safety best practices; emphasizing Required Organizational Practices and comprehensive patient safety management.
**Candidate Requirements**

* Bachelor's degree / Nursing license
* Preferred: 2 years’ experience in healthcare, clinics, or hospitals (clinical setting)
* Preferred: Participation in accreditation, certification, and categorization processes.
