Job Summary: Comprehensively navigate oncology and Bone Marrow Transplant (BMT) patients, coordinating timely, safe, and continuous care. Key Responsibilities: 1. Comprehensive coordination of the care pathway for oncology and BMT patients 2. Management of administrative barriers and patient/family education 3. Collaboration with multidisciplinary teams and continuous follow-up **Description** **Offer** -------------------------- Comprehensively navigate newly diagnosed oncology and Bone Marrow Transplant (BMT) patients, coordinating timely, safe, and continuous care. * Navigate newly diagnosed oncology patients from confirmed diagnosis through treatment initiation. * Coordinate the care pathway for BMT candidate and recipient patients. * Manage appointments, procedures, hospitalizations, and required examinations to ensure continuity of care. * Identify and manage administrative barriers that may delay treatment initiation or the transplant process. * Provide education and guidance to patients and their families throughout the diagnostic, therapeutic, and transplant processes. * Coordinate with Hematology, Oncology, Blood Bank, Hospitalization, Admissions, Auditing, Insurance, and other involved departments. * Conduct telephone follow-ups and provide accompaniment during various stages of care. * Document interventions performed and monitor indicators related to timeliness, continuity, and patient experience. * Participate in continuous improvement projects related to institutional oncology navigation. **Requirements** **Candidate** ------------------------------ * Licensed Nurse specialized in Oncology, registered and licensed. * Minimum 2 years of experience in oncology or onco-hematology units. 3 * Knowledge of clinical processes, care continuity, and management of complex patients. * Excellent communication, organizational skills, and ability to coordinate multidisciplinary teams. * Prior experience in patient coordination, support programs, or clinical navigation is desirable. * Prior experience with onco-hematology patients or Bone Marrow Transplant (BMT) is desirable. * Proficiency in Microsoft Office and health information systems. **Selection** **Process** ---------------------------- The selection process is conducted via Aira — a recruitment platform designed to enhance your application experience.
