We are currently seeking **01 ASSISTIVE SOCIAL WORKER** for Alberto Barton Hospital. **RESPONSIBILITIES** * Conduct social interventions for patients assigned to the Hospital Complex who belong to sectors with higher vulnerability (minors at risk, individuals exhibiting suicidal gestures, pregnant or postpartum women), patients in end-of-life stages, those enrolled in the PCT program, and other cases of social abandonment. * Carry out social intervention following these steps: 1\) Initial case assessment. 2\) Family socioeconomic evaluation. 3\) Identification of social diagnosis and determination of whether reporting to entities and/or institutionalization is required (e.g., Public Prosecutor’s Office, UPE, CEM, MIMP, etc.). 4\) Case follow-up. 5\) Conclusion of social care (case closure). * Provide efficient, high-quality care, guidance, counseling, support, and accompaniment to patients and their families, in compliance with current legal and institutional regulations. * Intervene in social cases arising in outpatient, emergency, and inpatient services (Hospital). * Coordinate with regional and governmental entities responsible for protecting socially vulnerable patients. * Conduct home visits. * Manage documentation through the Medical Directorate in cases requiring institutionalization or reporting to the Public Prosecutor’s Office, UPE, CEM, MIMPV, etc. * Address high-social-risk cases. * Manage documentation with external entities in cases of patient abandonment. * Provide counseling to patients and their families. * Deliver high-quality care to patients and, where applicable, to their families. * Transport patients and their families to shelters in cases of social issues. * Propose, according to professional competence, responses to inquiries, requests, complaints, and claims. * Other duties assigned by immediate supervisor. **REQUIREMENTS** * University degree in Social Work (licensed and registered) * Diplomas or courses in Family and Community Care, Social Diagnosis, Social and Socioeconomic Assessment Techniques, and Basic CPR * Minimum 3 years of experience in clinics, hospitals, or health centers * Availability to work in Callao (Alberto Barton Hospital and Polyclinic) **BENEFITS** * All statutory benefits. * Ongoing training. * Discounts with educational institutions, oncological insurance, vendor visits, and more. * Work for an internationally accredited company holding ISO and Joint Commission International (JCI) accreditation—one of the most prestigious global healthcare accreditations. *Our company is committed to equity and inclusion because we believe every person is unique and different.
